Waiver Summary Text:

NRG Track Club Informed Consent and Waiver Statement (For Minors)

 

By registering my child for NRG Track Club, I acknowledge that I am the parent or legal guardian of the participant and that I am voluntarily enrolling them in an athletic training program that includes physical activity, conditioning, and competitive events.

 

I understand that participation in track and field activities carries inherent risks, including but not limited to sprains, strains, fractures, dehydration, and other potential injuries. I affirm that my child is physically able to participate and has no medical conditions that would prevent safe involvement.

 

I agree to provide accurate and complete medical information during registration, including any allergies, asthma, medications, or other health concerns that coaches or staff should be aware of. I authorize NRG Track Club staff to seek emergency medical treatment for my child if necessary, and I understand that I will be contacted as soon as possible in the event of an emergency.

 

I understand that photographs and/or videos may be taken during practices, events, or competitions, and I grant permission for NRG Track Club to use these images for promotional, educational, or informational purposes, including but not limited to websites, social media, flyers, and newsletters. I understand that my child's name will not be used without my additional written consent.

 

I hereby release and hold harmless NRG Track Club, its coaches, staff, volunteers, and affiliated organizations from any and all liability, claims, demands, or causes of action that may arise from my child's participation in the program.

 

By completing this registration, I confirm that I have read, understood, and agree to the terms stated above, and I give my full consent for my child to participate in all NRG Track Club activities.
